From signal to action: a traceable care handoff

What a good handoff looks like when the signal, the decision and the documentation stay together.

A handoff fails when the next person has to reconstruct the story. The signal is in one system, the decision in a message and the documentation in a note written later. The patient experiences the delay; the team experiences the rework.

A traceable handoff has three properties: the signal is visible, the decision has an owner and the outcome returns to the same record. That is the whole pattern. The interface can be calm because the underlying chain is complete.

We design every alert with a destination. If there is no action, it is not an alert — it is a dashboard metric. The difference matters to a care team at 02:00.
