The principle

A signal is only useful if it changes what happens next.

The Docs4D platform turns patient signals into a shared clinical picture: what changed, why it matters and who needs to act. Not another dashboard. A clearer next step.

The patient journey

From first signal to next action.

One patient record. Four moments where the next decision matters.

01

Intake

The visit starts before the appointment.

Structured onboarding and consent create a baseline the care team can trust.

02

Triage

The risk is visible before the next appointment.

One signal moves to the top of the list. The team sees the context, not just the alert.

03

Monitor

The picture stays current between visits.

Vitals, adherence and patient-reported signals continue to update the same record.

04

Intervene

The next step is documented and traceable.

Escalation, documentation and follow-up live in the same record as the signal that triggered them.

The platform

One platform. Every care setting.

Start with the patient app, the clinical workspace or the workflow layer. They share one patient record and one traceable care logic.

Patient engagement

Care that continues between visits.

Patients get one calm place for monitoring, education, adherence and assistance. Care teams get signals they can act on.

  • Daily vitals and symptom check-ins
  • Care plans and education in the moment
  • Escalation that reaches the right person

Two views. One record.

The care team sees the whole picture. The patient sees the next step. Two sides of the same record, without losing the thread.

Why we built the Docs4D platform

“In medicine, clarity is not a luxury. It is essential.”

We built the Docs4D platform to give clinicians back the one resource they never have enough of: time for their patients. Every workflow, every signal and every note should make the next decision clearer — not add another screen.

Questions, answered

The things care teams ask.

If you do not see your question, talk to a clinician on our team.

What is SuperDoc?

SuperDoc is a conversational AI avatar for patient interaction, one product on the Docs4D platform alongside the clinical workspace and monitoring. The platform connects the patient app, the care-team workspace and the workflow engine around one traceable patient record.

How does the Docs4D platform fit our existing workflows?

The Docs4D platform is designed around clinical pathways, not a rip-and-replace. We map your current workflow, identify the highest-friction handoffs and start with one measurable care pathway.

Is the Docs4D platform a medical device?

Regulatory status depends on the product and intended use. Current status is shown in the Trust Center and should be reviewed with our regulatory team before any procurement decision.

How is patient data handled?

Data protection is designed into the Docs4D platform: role-based access, audit trails and EU-based infrastructure. A data processing agreement is available for evaluation.

What does implementation look like?

We begin with a 30-minute discovery call, then a focused workflow review. A first pathway can be configured without a large IT project; integrations are scoped separately.
